Employees complete a "Request for Employee Computer Accounts" form and return to the Department of Information Technology Miller Hall room 115.
Adjunct faculty and other remote employees may fax their completed forms to (308) 432-6471 or scan a completed form and e-mail it to helpdesk@csc.edu.
